Rapid advances in high throughput genomic technologies and next generation sequencing are making medical genomic research more readily accessible and affordable, including the sequencing of patient and control whole genomes and exomes in order to elucidate genetic factors underlying disease. Over the next five years, the Human Heredity and Health in Africa (H3Africa) Initiative, funded by the Wellcome Trust (United Kingdom) and the National Institutes of Health (United States of America), will contribute greatly towards sequencing of numerous African samples for (...) biomedical research. Background: Clinical genomic implementation studies pose challenges for informed consent. Consent forms often include complex language and concepts, which can be a barrier to diverse enrollment, and these studies often blur traditional research-clinical boundaries. There is a move toward self-directed, web-based research enrollment, but more evidence is needed about how these enrollment approaches work in practice. In this study, we developed and evaluated a literacy-focused, web-based consent approach to support enrollment of diverse participants in an ongoing clinical genomic implementation study. (...) Methods: As part of the Cancer Health Assessments Reaching Many (CHARM) study, we developed a web-based consent approach that featured plain language, multimedia, and separate descriptions of clinical care and research activities. CHARM offered clinical exome sequencing to individuals at high risk of hereditary cancer. We interviewed CHARM participants about their reactions to the consent approach. We audio recorded, transcribed, and coded interviews using a deductively and inductively derived codebook. We reviewed coded excerpts as a team to identify overarching themes. Results: We conducted 32 interviews, including 12 (38%) in Spanish. Most (69%) enrolled without assistance from study staff, usually on a mobile phone. Those who completed enrollment in one day spent an average of 12 minutes on the consent portion. Interviewees found the information simple to read but comprehensive, were neutral to positive about the multimedia support, and identified increased access to testing in the study as the key difference from clinical care. Conclusions: This study showed that interviewees found our literacy-focused, web-based consent approach acceptable; did not distinguish the consent materials from other online study processes; and valued getting access to testing in the study. Overall, conducting empirical bioethics research in an ongoing clinical trial was useful to demonstrate the acceptability of our novel consent approach but posed practical challenges. (shrink)

Experimental philosophers have recently questioned the use of intuitions as evidence in philosophical methods. J. R. Kuntz and J. R.C. Kuntz (2011) conduct an experiment suggesting that these critiques fail to be properly motivated because they fail to capture philosophers' preferred conceptions of intuition‐use. In this response, it is argued that while there are a series of worries about the design of this study, the data generated by Kuntz and Kuntz support, rather than undermine, the motivation for the experimentalist critiques (...) of intuition they aim to criticize. Although still deeply influential in the nineteenth century, it has been unjustly neglected in modern times. The work contains a fascinating collection of views on scientific terminology and taxonomy, the application of the logical methods of collection and division to science, the axiomatization of science, and the structure of causation. Consequently it is a key text in later Greek philosophy of science. R. J. This paper deals with Aristotelian and Stoic logic. In the first part the author writes about the history of logic and shows, why Stoic logic had not been studied properly from the Middle Ages up to the beginning of the 20th century, when an increasing interest in the study of Stoic logic is visible. The paper describes the character of Aristotelian and Stoic logic respectively. Stoic logic is first introduced as a system of propositional logic. On this basis a complementarity (...) between the two logical systems of Antiquity is stated. The ways to support the thesis that Stoic logic involved some features of predicative logic are shown at the end of the first part. The ways in which the two rival logical systems were perceived by the authors of the first centuries A. D., namely Galen and Boethius, is described in the second part. Galen is seen as the first to develop an objective synthesis of the two systems. In his Eisagogé dialektiké he tries to show the goals of both rival logical systems. Boethius, in his De hypotheticis syllogismis, is also combining the Aristotelian and the Stoic logic. However, his synthesis is different from Galen's because Boethius probably forms the synthesis unknowingly and indirectly through compiling his confused sources. (shrink)

An ancient doctor who advocated the therapeutic benefits of wine and passive exercise was bound to be successful. However, Asclepiades of Bithynia did far more than reform much of traditional Hippocratic therapeutic practice; he devised an extraordinary physical theory which he used to explain all biological phenomena in uniformly simple terms. His work laid the theoretical basis for the anti-theoretical medical sect called Methodism. For his trouble he was despised by his intellectual progeny and, more importantly perhaps, by Galen. (...) None of his work survives intact, but copious ancient testimonia relating to him allow us to reconstruct many details of the theory. His ideas offer us a fascinating glimpse of how Hellenistic philosophy and medicine interacted, and provide an introduction to one of the most intriguing doctrinal disputes in Greek science. Galen of Pergamum was the most influential doctor of later antiquity, whose work was to influence medical theory and practice for more than fifteen hundred years. He was a prolific writer on anatomy, physiology, diagnosis and prognosis, pulse-doctrine, pharmacology, therapeutics, and the theory of medicine; but he also wrote extensively on philosophical topics, making original contributions to logic and the philosophy of science, and outlining a scientific epistemology which married a deep respect for empirical adequacy with a commitment to (...) rigorous rational exposition and demonstration. He was also a vigorous polemicist, deeply involved in the doctrinal disputes among the medical schools of his day. This volume offers an introduction to and overview of Galen's achievement in all these fields, while seeking also to evaluate that achievement in the light of the advances made in Galen scholarship over the past thirty years. Voltaire, of course, is poking fun at such conceptions; and, no doubt, in their extreme sanguinity as well as in their apparent (...) imperviousness to devastating empirical counter-evidence, they do seem to be eminently risible notions. In the face of them we might be tempted to abandon ‘métaphysico-théologo-cosmolonigologie’, and to agree with Candide that ‘Cela est bien dit, mais il faut cultiver notre jardin’. (shrink)

What, for Galen, are powers, and how are they to be properly individuated? The notion of a power or capacity does a great deal of work in Galen. As in Aristotle, the concept of a dunamis is tightly linked with that of an energeia, but these are not simply logical abstractions. Rather the natural energeiai are the basic functional activities of the animal body and its parts, and just as health consists in proper functioning, so disease is defined (...) as ‘damage to one of the natural energeiai of the body’, and these activities are damaged when something interferes with its related dunamis. Here I try to make sense of the apparently very different things Galen says regarding dunameis. For example, he says that they do not inhabit our bodies as we do our houses; that is, presumably, they are not substantial or hypostasized; rather they are properties of us. This book is a new edition of a short but fascinating treatise by Galen on causal theory. This text survives only in a Latin translation of the fourteenth century, and it is this which appears here. The volume also contains the first translation of the treatise into any modern language, and the first philosophical commentary thereon. The commentary ranges widely in Galen's voluminous œuvre, and compares his views with those of other ancient theorists. Determining what has gone wrong in a malfunctioning body and proposing an effective treatment requires expertise. Since antiquity, philosophers and doctors have wondered what sort of knowledge this expertise involves, and whether and how it can warrant its conclusions. Few people were as qualified to deal with these questions as Galen of Pergamum. A practising doctor with a keen interest in logic and natural science, he devoted much of his enormous literary output to the task of putting medicine on (...) firm methodological grounds. At the same time he reflected on philosophical issues entailed by this project, such as the nature of experience, its relation to reason, the criteria of truth, and the methods of justification. Galen Strawson’s Basic Argument is that because self-creation is required to be truly morally responsible and self-creation is impossible, it is impossible to be truly morally responsible for anything. I contend that the Basic Argument is unpersuasive and unsound. First, I argue that the moral luck debate shows that the self-creation requirement appears to be contradicted and supported by various parts of our commonsense ideas about moral responsibility, and that this ambivalence undermines the only reason that Strawson gives for (...) the self-creation requirement. Second, I argue that the self-creation requirement is so demanding that either it is an implausible requirement for a species of true moral responsibility that we take ourselves to have or it is a plausible requirement of a species of true moral responsibility that we have never taken ourselves to have. Third, I explain that Strawson overgeneralizes from instances of constitutive luck that obviously undermine moral responsibility to all kinds of constitutive luck. (shrink)

I am presently concerned not with the unresolved crux in verse 144, but with the medical reason for the death of the glutton. Galen , quoted by Mayor, warned that one should not bathe after eating να μ μραξις κατ νερς κα παρ γνηται. More recently, Courtney ad loc. has quoted Persius 3.98ff. and has attributed the death to ‘apoplexy’, which in more modern parlance (...) is called a ‘stroke’ or a ‘cerebral haemorrhage’. What Persius and Juvenal are actually describing is not a stroke but what was formerly known as ‘acute indigestion’ and is now called a ‘heart attack’, as indeed ought to have been obvious from ‘nescio quid trepidat mihi pectus’ at Persius 3.88, and ‘tange, miser, uenas et pone in pectore dextram’ at 3.107. As Duff says, ‘the natural and ordinary time for bathing was just before the cena, but the gluttons of this time had discovered that digestion was temporarily promoted by the unhealthy practice of bathing in very hot water immediately after the meal’. Modern medical research has shown why this practice was very unhealthy indeed. As a meal is digested, the pulse rate is elevated, and bathing in hot water increases it even further. The consumption of alcohol, such as that described at Persius 3.92–3 and 99–100, would further accelerate the heart beat. The synergistic effect of these three circumstances, digesting a heavy meal, metabolising a large dose of alcohol, and bathing in hot water, was liable to cause a heart attack in an overweight man whose arteries were clogged with cholesterol.
